Casa Colina Conducting Community Health Needs Assessment

Casa Colina Hospital and Centers for Healthcare is conducting a Community Health
Needs Assessment. The Community Health Needs Assessment identifies and assesses
the health needs of the community served by the hospital. An important component
of this assessment is to listen to community opinions regarding health and social
issues.

The hospital invites you to participate in an online survey for our Community Health Needs
Assessment. The survey will help it to better understand the challenges that you and
your household face. Your answers are anonymous, and you have the option to
answer some or all of the questions. Your answers, along with those of others, will
help us to develop our strategic community outreach plan that we will implement for
the next three years.
